To start the FEPSA application process, complete the following form. In order to be eligible for membership, you must be a licensed insurance agent.

When you click the submit button, you will be taken to our payment portal. Once we have reviewed your application and your payment has been processed, we will activate your membership and send you information on how to begin using your valuable benefits.
